About HART
The Healthy Aging Response Team is here to help our aging community members (50+) and adults with disabilities (18+) navigate through available community resources; We take the Stress out of the way and support participants to thrive through every life stage.
Empathy, Compassion, Respect, Trustworthiness and Patience are characteristics and qualities of our staff and volunteers throughout Daly City Partnership. HART is a collaboration of friendly peers working towards a common goal: To assist our aging and disabled participants navigate community resources and the health care system. We seek to support participants to engage in the programs that will keep them active in their community and fulfilled throughout their lives.
